SMART Social Care Support is a Children and Familes, 16+ and Adults social care service provider. Our name is derived from and inspired by the 5 pillars of Care Planning. We work in partnership with local authorities, universal services and other partner agencies to draw up Specific, Measurable, Achievable, Realistic and Timely action plans to meet the identified needs of service users. Our staff is well trained to deliver safe and good quality care.
