SMART Social Care Support is a social care service provider for vulnerable young people and adults. Our duty is to work with local authorities and other agencies to safeguard and promote the welfare of vulnerable young people and adults entrusted in our care. Our name is derived from and inspired by the 5 pillars of Care Planning – where professionals come up with Specific, Measurable, Achievable, Realistic and Timely actions to meet identified needs of a service users. Our staff is well trained to deliver safe and good quality care.
